Home Health Aide (Full-time/Part-time)

Provide health related services in the patients home as outlined in the client's plan of health care, authorized by a physician and directed by a registered nurse or licensed therapist. Assist with personal hygiene, housekeeping and/or other related supportive tasks to a client with health care needs in his/her home. In addition, HHA's are required to submit necessary documentation including charting tasks and observations on time/activity sheets. Job Qualifications: Possess HHA Certification or CNA licensure. Be free from any health impairment that is of potential risk to the patient, family or to the employee, or that may interfere with the performance of duties. Be able to read and write english at an 8th grade level.

Skilled Nursing (Full-time/Part-time)

Administer skilled nursing care to patients in the home setting. Teach and supervise the family and other members of the nursing team. Perform these services in accordance with the physician's orders and under direction and supervision of the Director of Nursing/Program Managers and Clinical Care Manager/Team Leader.
